Post-Diploma Certificate in Chronic Disease Nursing Practice and Management Module Synopsis Case Management, Resource and Tools This module provides an understanding of discharge planning and the various resources available for the patients. Health Education & Teaching The module focuses on developing participants' knowledge and skills in providing patient education. It provides a holistic orientation which values physical, mental, social and spiritual dimensions of health and their inter-relationships. The major aspects of education strategy and pedagogy in chronic diseases will be covered. Nursing Research This module covers a range of theoretical and practical aspects of research. This includes proposal writing, research critique and understanding the use of statistics in research. Emphasis on Evidence Based Practice will enhance awareness in promoting optimal care in clinical setting. Counselling for Behaviour Change This module provides an understanding of the fundamental spirit and principles of motivational interviewing that could be applied in helping clients who are ambivalent to change. Medical Informatics & Financial Counselling The module is designed to equip participants to be effective users and managers of health information resources. It provides an understanding of these information systems and their appropriate use within a healthcare setting. Participants will learn to identify information needed, financial counselling and will understand how data is used to make effective healthcare decisions.
