Post-Diploma Certificate in Advanced Clinical Management of Chronic Diseases Module Synopsis Advanced Pharmacology This advanced module is expected to deliver a more extensive study of pharmacology in knowledge of pharmacokinetics and pharmacodynamics of various classes of drugs. It includes studies of drugs that are commonly used in patients with Chronic Diseases. It covers drug interactions, contraindications and side effects with emphasis on nursing consideration and patients' education. Advanced Pathophysiology of Chronic Diseases This advanced module provides an in depth understanding of pathophysiology that will support clinical decision making on diagnosis and treatment of chronic diseases. These include Hypertension, Hyperlipidaemia, Stroke, Coronary Artery Disease, Diabetes, and Heart Failure, Renal failure, Chronic Obstructive Pulmonary Disease (COPD}, Osteoporosis, Depression, Dementia and Schizophrenia that are prevalent in patients with chronic diseases will be discussed. The programme is designed for nurses to advance their clinical practice, develop their professional career and enhance their knowledge of disease process and management with updated clinical guidelines. Health Assessment This module provides an understanding and comprehensive foundation on the principles of health assessment in nursing practice. It introduces the skills necessary to obtain data through health history, physical examination together with identifying relevant laboratory data and monitoring adjuncts. Rehabilitation & Non-Cancer Palliative Care The module optimizes the management and care of patient with complication of chronic disease of the pulmonary, cardiovascular system, in addition to stroke and cardiac rehabilitation. The module approach to rehabilitation will be patient centered.
